Rehosting Quartus License

I need to rehost my Quartus license to my new workstation.

However I can't access Intel Self-service Licensing Center because of an Access Denied error. Support was not able to help with this error and directed me to post my query to this forum.

Please advice how to regenerate the license file.

    Company Address :

    Rehost/change details

    OLD and NEW NICID(Mac Add) :

    Also provide the previous license.dat file
