We purchase the above p/n from an authorized distributor but the marking on the device is not completely clear, we have some concern about quality issues, anybody can validate the below lot code: ...

You'll have to talk to Altera on that one. However, something to bare in mind is that these parts used to be manufactured for Altera by STMicro, which became Numonyx, which became Micron. So, you might see something other than an Altera part number on there.
