USB Blaster works only once per Quartus installation

This problem with USB Blaster seems to be a little different from any of the other posts and so I have not found a solution from reading the many posts relating to USB Blaster problems but I have tried and solution that I found that was even remotely related.

In my case the USB Blaster hardware only shows up once in programmer hardware setup, and only after the initial installation of Quartus. If I reboot my PC the USB Blaster does not show anymore even though it is displayed in the device manager. Uninstalling the driver and reconnecting the DE1 development board brings up the "found new hardware" dialogue box. Installing the driver works fine but the USB Blaster never shows in the hardware setup in the programming window (I have tried this multiple times). the only way i have been able to get this to work is to uninstall quartus and reinstall it again - and not reboot my pc. I can restart Quartus, just not reboot my PC. The DE1 board works fine on another PC (not one of mine) running 11.1 sp2

I started with Quartus 8.x about a year ago and as far as I recall I didn't have any problems but I have not used it for a year and Quartus was uninstalled. Over the last month I have tried Quartus 8.x, 10.1, 12.0, 12.0 sp2 and now 11.1 sp2 I have tried installing and operating as Administrator. I have also uninstalled all the USB device drivers including the hidden ones.

I am running Windows XP pro sp3 32bit

It appears the problem lay in the fact that Zone Alarm was blocking jtagserver.exe without reporting it. In the firewall settings it was automatically configured (I had Zone Alarm configured to notify me of changes) to request permission each time it was accessed - but no requests were actually made. I uninstalled Zone Alarm and the USB Blaster appeared in the "installed hardware" in the programmer. I rebooted my PC several times to be sure.
