I agree that things are a bit complicated since Altera switched to QuestaSim Altera Starter Edition a few years ago. But it's clearly stated in installation instructions that Questa uses a separate license file indicated by the SALT_LICENSE_SERVER environment variable. License files are readable text, a Questa license should contain a feature "intelqsimstarter".
A free QuestaSim ASE license can be obtained from Self Service Licensing Center if you don't already have it. Currently the SSLC license generator seems to have issues, hopefully fixed soon.

Hi Frank,
So before making this post, I did actually get a separate license for the Questa - FPGA Starter Edition (I have the 2025.2 version) and didn't see much success applying it (followed the instructions and setup the environment variable appropriately). I'm not sure if that is what you meant by a "QuestaSim ASE license." I did not see the "intelqsimstarter" line in it either.
I've been using this webpage as my guide 5.1.2. Questa* Altera FPGA Edition and Questa* Altera FPGA Starter Edition Software License • Altera FPGA Software Installation and Licensing • Altera Documentation and Resources Center.
I did download that separate license today, so maybe those issues are to blame.
