Reinstalling Quartus II 10.0 & 10.0 SP1 with Hardcopy III device support seemed to resolve the issue for me.

I also think the problem can be narrowed down to when I use the DE3 System Builder tool included on the DE3 CD. A bare bones DE3 module contains clocks and the proprietary IOV IP. This needs to be confirmed.
