Error Enrolling for the FPGA Self-Service Licensing Center

When I do self-service license generation, I met the following problem.

First of all , I log into my Intel account and visit into this page.

Choose with steps"Enroll for Intel® FPGA Self Service Licensing Center (SSLC)"

After submitting "Program Enrollment"

I went to the "Submit" page.

After confirming the information, I pressed the "submit" button.

The following steps did not continue, so it stopped at this state.
