I also had problems with the flash programmer in NIOS 6.1 IDE and I reported it to Altera.
There's a bug in flash programmer, which will be fixed into NIOS 7.0.
The workaround for me was to add a "short" name into the board name in SOPC.
I.e. the board name should be like "Full name (Short)". I had only "Full name" and Flash Progammer gave an error when trying to create a new lauch configuration.
