In my experience, and this isn't conclusive.. but when the programmer tries to reset the programmer and it fails the first time, it's likely a hardware issue, not software. As in the avalon bus is locked up.. if it's stuck in a loop or something, it can usually reset it the first time. What hardware components do you have in your system.
