To use JTAG you must use the USB blaster rev B. There are some problems when you are using the rev A. If you are using a custom board, pay a real attention on the JTAG connection : pullups, wire size. To make my JTAG working I had to change pullups from 10KR to 1KR and add a 32R serial on the JTAG clock.
