    "Let start with what are the other thing your observed during the failure Nios II Boot?"

    I follow the instructions as listed in the document I linked above. The NIOS does not blink the LEDs as I programmed it to do. I have no other observations. If you have knowledge of how to debug further, let me know and I can provide more information.
