Quartus JTAG programmer error

A single device EPM3128ATC144-7N is attempted to be programmed with JTAG through USB blaster but Quartus always gives this error:

Warning: Uncertain JTAG chain. Detected 1 device(s)

Info: Device 1: UNKNOWN_162701B9

The connections have been thoroughly tested as OK and its all running off 3.3V - Can anyone explain this??
