Max 2 IC occasionally gets destroyed

In my hardware environment, a "Max 2" IC occasionally gets destroyed - it pulls down the 3.3 V power supply. What can be the cause for this? Can it be anything else than electrostatic discharge? The ...

I have the same problem on max V CPLD. All the power supply pins are short-ciruited to GND pin. I suspect te GND loop but it looks like it's not really repeatable, so far 3 CPLD have been burned during programming.
