How to get the RMA to return the sample for analysis?

we have FPGA device (Stratix 10) failed functional test Q'ty 1 pcs from product BOSS HOGG under failure symptom "TPO667_4 SILAB PLL CLOCK CHECK TEST" we need to return FPGA device back to Intel for analysis. please help to review.
