Under what conditions would a JTAG verification failure occur?

Update:

We have tried to use the power rail (5V with filter network) of working device to power up and program the non-working CPLD (after isolating the existing power rail to non-working CPLD). And we are seeing that the Program/Configure and Blank-check operations are now intermittently successful (say out of 10 attempts, at least 5 - 6 times successful). The filtering of 5V is attached in the screenshot.
