How to generate case id for Failure Analysis?

I'm Siti Nurain Supplier Quality Engineer for Juniper project in Flex Penang.

Flex encountered high fallout for this PN 310-055414, MPN 5SGXEA4H2F35I3NCV and want to send back to Altera for Failure Analysis. Please help to create case id for this feedback. Looking forward your respond.
