When I open the quartus it prompts me a window "Evaluation Mode" when I select the 4th option of specifying the licence file it works. But it prompts the same message again when I restart.

Could you help check your LM_LICENSE_FILE environment setup. As part of the license setup process, you need to specify the location of the license file using the LM_LICENSE_FILE system variable. You need to set your LM_LICENSE_FILE environment variable to point to the license that Intel provide. c:\intel\license.dat or something similar
