I basically try the setting in the manual.

I used 2.5 as reference voltage. 

1 0.2
2 0.5
3 0.8
4 1.1
5 1.4

I used dual ADC IP core.

I already figure out the problem.

I remove the defparam for reference_voltage_sim.

Yet, I'm still puzzled does it mean 65536 = 2.5V?
