Failure to launch Questa Intel FPGA

In Quartus software, go to Tools -> Options -> EDA Tool Options -> Questa Intel FPGA and specify the paths:

<quartus_installdir>/questa_fse/bin (Linux)

<quartus_installdir>\questa_fse\win64 (Windows)
