Hello Asaf,
If the device is not output from POR, do you have the oscilloscope capture the power rails ramping up?
Any abnormal voltage ramp up during power up? Especially all the power rails monitored by POR circuitry.

Thanks for your reply,
We checked Cold start up on other PCB and it pass successfully, so maybe it is assembly issue of specific PCB, we send it for examination and preheat of the FPGA and we will check it again.
See attached oscilloscope capture:
- Green - PMBus (3.3V), Red -VID voltage, Blue - 1.8V, Yellow - nConfig - it good power up, you can see transition on the PMBus.
- Red - VID, Yellow - nConfig, Green - SDM clock, Blue - Nstatus.
I don't see the SDM start the configuration prosses
