I definitely have USB blaster installed correctly.
Quartus II 9.1 build 222 no service pack. The board name is EP4S100G2F40I1N.
This was the first time I opened Quartus and do not have an active licence. Does this matter for the demo? The quick start guide does not mention opening Quartus at all.

How did you install the USB blaster? I suspect that the demo cannot communicate the board because of the driver problem. Can you show " list of things to check and they all seem ok" you mentioned?
The quick guide does not mention Quartus license because you are provided the demo.sof file already, all you need is the Quartus II programmer that enables you to upload the demo.sof file to FPGA for running. And doing this requires no license.
PS: what is your system OS?
