Kindly refer to the feedback below:
1. What is the failure symptom? Please elaborate the failure symptom in detail.
Customer reported that copper SFP was inserted in the device. it seems to be a hardware failure and no information can be retrieved.device is hard down. By further test the customer board and confirm the board failed to boot up and failed Diagnostic test. We suspected on IOFPGA U45 since there is no progress of POSTCODE led after TTL_RESET_L is high. We measured voltage and clock supplied to IOFPGA U45, all good. We also measure impedance for IOFPGA_CPU_PCIE signals compared with KGB no abnormality. Perform X-ray on IOFPGA U45 and there is no abnormality on solderability. Perform replacement on IOFPGA U45 and perform online test also offline diag test, the result is passed. Through isolation steps, the root cause is fixed to IOFPGA U45.
2. When did the failure happen? How did you discover the failure?
Failed found once try to boot up the board. Suspected on IOFPGA U45 since there is no progress of POSTCODE led after TTL_RESET_L is high
3. How did you determine the failure? Please elaborate the procedures.

4. Does the failure unit ever working before failure?
Yes. It work before failure.
5. Did they violate solder re-flow temperature profiles, moisture sensitivity? Please provide the re-flow temperature profiles.
No. This part did not undergo any reflow process before this.
6. Did you swap the failure device to a known good board? Is the failure following the device or board?
No. However, we will swap the failure device on another good board to confirm its failure first if ALTERA is willing to the support the FA for this part.
7. Is this a prototype build or volume/mass production?
Volume/mass production. However, it is a field returned part from our end customer.
8. Kindly provide quantitative investigation result that could proof the failure is Intel FPGA induced.
By replace this part with another good part, the board can function well now and work normally.
