USB-Blaster not recognized

Why is my USB Blaster download cable does not recognized with QuartusII 11.1 sp2, WIN7 64bit. Drivers installed correctly.

TDO always stuck at 0 error

We have a USB Blaster Rev C. JTAG Programmer

whenever we are trying to detect the target device using UrJTAG Shell

It is displaying the same error "TDO always stuck at 0"

We have checked the following in an oscilloscope

1. TCK- We can observe for 1 second and after its being turned off similar is the case for TMS & TDI

2. TMS

3. TDI

What could be the possible problem??
