block diagram output pin problem

Using QIIWE 9.1Sp2 on ubuntu 10.04

having a weird issue with output pins in a block diagram, when i create or update the symbol file for the block diagram the output pins dont show up on the symbol and there are warnings generated:

Warning: Ignored port pin_name in symbol test -- port is illegal

for each output pin

ive tried many different connection setups and pin names nothing seems to work i cant seem to get the output pins working, the input pins do work fine and show up on the symbol correctly

im very new to the software so not sure if theres something obvious im missing
