Your log is not readable like this, so it is hard to tell what is happening. Have you followed Altera's instructions in the OpenCL Getting Started Guide? Also what version of Windows are you using? Others have reproted that correct driver installation on Windows 8/8.1/10 requires disabling driver signature enforcement.
