sd card read function

has anyone worked with sd card's read function? im trying to read a very simple hex file but i get the wrong data. I've saved the file in FAT16 format on sd card and its format is recognized properly...

How do you read data from SD card?

Do you use an OS or a specific file system manager? Or maybe raw SD data read?

How did you save the file? Same board or another device (i.e. your PC) ?
