Honored ContributorIf you don't mind a quick learn of Altera's proprietary AHDL language, that is probably the quickest way to put your truth table in a design (search on "truth table" in Quartus).
Once that simple file compiles, you can do "File... Create/Update ... Create Symbol file for current files," and this will create the block diagram symbol that you want.
