Most likely is the JTAG pins left floating which stop the device from working, as Pete has mentioned. Also make sure the bottom metal pad on the FPGA package is grounded. Please check that each and every FPGA pin follow the rules listed in this document: https://www.altera.com/content/dam/altera-www/global/en_us/pdfs/literature/dp/cyclone3/pgc-01003.pdf
