Enpirion EM2120H Vout OV OC error on startup

When the control signal is pulled high, the EM2120H reports a Vout OV and OC error and stop producing a Vout. This happens almost immediately with out a substantial increase in Vout or Iout. the PCB has been checked for short circuit faults between Vout and ground. Is this a behavior that is know to happen with specific design implementation issues? Current design mirrors the example design from the datasheet, but with Rvset at 576ohm (3.3Vout), R1DIV 2K and R2DIV 1K.
