Cyclone III configuration

I'm about to spin my Cyclone 3 board and want to be sure I get the configuration stuff wired correctly. I'm using a JTAG connection to the USB Blaster and a Serial Config Device (active serial)...

I wonder, how you managed to generate so much trouble with a few designs. There must be a systematic problem, but unfortunately I have no clue what it is. I finished various EPC3 designs in the last months, several using AS, apart from minor Quartus SFL issues configuration was always fully operational.
