Could you try this link - https://licensing.intel.com/eval-free-licenses after clearing your browser cache and cookies.

I have created an account to request for the license for Questa-Intel FPGA Starter Edition.
But it looks like my account is not up yet. Till now I still can't get the license yet.
I tried with the link https://licensing.intel.com/eval-free-licenses. Still it brought me to the 3rd screenshot.
I attach some screenshot here for your reference.
