i need help with coursework

(I have Done the courswork by myselfe, without a help from anyone , just sample,,, I spent about 70 hrs to play with quartus ii and modelSim) Pleas take look in attachment. i need help...

thank you guys for not helping me to do my coursework :),,,, after 4 weeks, i can say , Now I am an Altera expert ....I can do all kind of simulations without an erros (with Full version) XD.
