Honored ContributorA possible reason is that your initialisation isn't read. If so, it could be seen from a simulation warning. Generally *.hex is required for ModelSim, and must reside in ModelSim project directory, that's where the modelsim.ini and *.hdp project files are located. The ROM content can be examined also in simulation memory window.
