glitich in the outputs

i am getting some glitiched in the outputs, i am not sure what is the reason of then glitich and how to get rid of them, the glitichs happen in the output that are not changing

thank you for the reply, as i have read 1 process template is not good choice , i just read it ins ome books that is why i did not use it.

about the second option , to make another process for the output, can you make it , would you show me how to implement it in my design , just the first process and its senstivity list and the clock
