FPGA flash issue

We are getting an error message while flashing FPGA via USB Blaster. Error message we receive are mentioned below.

1. Erase failed at offset 40000 (This address keeps changing)

2. Verifying 00000000 (0%) Verify failed

The MFD module used for flashing is 64MB module and snapshot of EPCS information programming log file

We are using cyclone III FPGA

EPCS signature is 0x15[/B]

epcs identifier is 0x010215

using epcs size information from section [epcs-010215]

device size is 4mbyte (32mbit)

erase regions are:

offset 0: 64 x 64k

epcs status is 0x00[/B]
