Cyclone III and FLash --need some help people

Dear Fellow Users

I need some help programing my FLash on my cyclone 3 development board. I am using a 1 MHZ clock, so timming should not be an issue. Please could you have a look at the attached pdf image as it shows my state machine. i am have no luck, it would be nice if some one could show where i am going wrong.
