the most important point is that freq_locked is not asserted, the question is what would cause that?

No signal on the receiver pins, incorrect reset sequence, etc.
Look at the attached document. It contains the reset sequence for a receiver-only design. I've done similar things on a Cyclone IV GX Starter Kit, and they are pretty much the same.
Use SignalTap to trace your reset controller and check that it looks the same (or at least similar).
