most likely the flash device you are using is not in the database and you have to add it as a new CFI Flash Device, see page 33 to 34 in the parallel flash loader ip core user guide (https://www.altera.com/content/dam/altera-www/global/en_us/pdfs/literature/ug/ug_pfl.pdf).
