Why does my state machine enter an illegal state?

Description

If your state machine has an asynchronous reset which does not meet recovery and removal timing requirements, the state machine may enter an illegal state.

Resolution

To avoid this problem, either fix the recovery and removal timing violations or turn on the Safe State Machine Logic option.  See Quartus II Help for the Safe State Machine Logic option.
