Does Altera provide a perfluorooctanesulfonic acid or perfluorooctane sulfonate (PFOS/PFOA) letter of compliance?

No, Altera does not provide a PFOS/PFOA letter of compliance.  All Altera® products have been free of PFOS/PFOA since 2009.
