Why does the Design Assistant fail to report a violation of Rule M104 for my design?

Description

Due to a problem with the Quartus® II software versions 9.0 and later, the Design Assistant may not flag violations of Rule M104 for designs that should trigger the rule check. Rule M104 should be triggered for designs containing finite state machines with deadlock states. A deadlock state is a state that does not have any transitions to another state except to loop back to itself.

This problem is scheduled to be fixed in a future release of the Quartus II software.
